Appointment & Payment
You are responsible for scheduling your appointment and arriving on time.
Fees for Services are due at or before the time Services are rendered, unless otherwise agreed in writing.
We accept HSA and FSA cards, but do not accept insurance for Services unless otherwise noted.

Medical Advice & No Guarantee of Results
The Services are provided as part of a medical practice. Your provider will explain risks, benefits, alternatives, and likely outcomes.

You represent that you have provided accurate medical history and that you will follow all instructions given by the provider.
Risks & Consent
By proceeding with Services, you acknowledge that you understand there are risks inherent in medical and aesthetic treatments (e.g., injection reactions, bruising, changes in sensation). You authorize your provider to perform the Services recommended. If you have any questions, you agree to ask your provider before treatment.
Personal Data & Communications
You consent to receive communications from us (phone, email, text) for appointment reminders, treatment plans, and clinic updates.
